Please print a copy of the Medical consent form, and complete ONE COPY PER CHILD. You will need to bring the completed AND SIGNED form to reception on the day of the camp, prior to the child's commencement.
Before completing the Enrolment Form, please read through our policies and procedures outlined below.
IMPORTANT:
In the event that your child is absent or unable to attend there will be no refund or deferred day.
Note: In the event that a camp can not be filled with required numbers, the camp may be canceled and staff will ensure either a refund or deferred camp date.
In the event of wet weather on the day of the camp, please be advised that the camp will still go ahead. Ensure that your child/ren have appropriate wet-weather attire such as rain jackets with hoods.
Children must be signed in and out each day by an adult. Sign in/out sheets must be signed at reception on the day of the camp.
Dragon Camp staff may take photos of your child during the camps for marketing purposes, to display on our website, or to provide to campers and their families as a memory of their camp experience. We request that you give permission on your Medical Consent Form so that we can take your child's photograph. If you do not want your child to be photographed, please indicate on the enrolment form.
Any child who sustains an injury or is involved in any incident whilst at the Dragon camps will be treated by our trained instructors, who hold senior first aid certificates. All accidents and injuries will be verbally reported to parents. Should your child sustain a serious injury whilst with the camp, we will contact you as soon as possible, and emergency care will be sought.
Dragon Camp Instructors hold blue cards for working with children, senior first aid certificates, and specialised credentials for the positions that they hold. Dragon Camp's senior instructor is a Certified Instructor with the International Martial Arts Alliance.
Fees for the Dragon Camps are to be paid by cheque/money order (make out to L&S Walters, post to 1859 Flagstone Creek Road, Upper Flagstone, QLD 4344) or direct money transfer (please contact us for account details).
Current Fees for 22-26 September 2008 camp are $200 per child.
Fees must be sent and received in full prior to the commencement of the Dragon Camp nominated. Payment will be confirmed by email/phone.
Please note: Failure to pay fees will result in the cancellation of your child's enrolment in the Dragon Camp.
Dragon Camps run from Monday to Friday from 9am to 3pm on the week of the camp.
Packed lunch, comfortable closed training/walking shoes, hat, water bottle, wet weather clothes (rain jacket with hood).
